If you submitted an Electronic Request for an Intake Form, we will begin the process of trying to find you a provider. If we are able to connect you with a provider, we will respond to you by email and begin the scheduling process. If we are not able to connect you with a provider, our Intake Coordinator will send you an email notifying you shortly.
If we are not able to offer you services at this time it is most likely due to scheduling or insurance provider enrollments conflicts. Please note that all submitted health information you will be protected whether or not we are able to provide you services .
If you have submitted an adult or minor child health questionnaire & consent form your clinician will be ready for your upcoming appointment.
